FINANCIAL ARRANGEMENTS
Your responsible amount of payment is expected on the day of treatment.
We accept cash, personal checks, and credit cards (Visa, MasterCard, American Express, and Discover).
There are outside payment plan options available to those who qualify. If you are interested in details please inquire prior to your scheduled appointment and information will be provided.
COLLECTION OF BALANCES DUE SHOULD THEY OCCUR
A statement will be sent after all insurance claims have been processed, paid and/or closed. These statements will include any unpaid balance to date. If you have ANY questions as to this balance due, please contact our office manager for assistance.
If any balance due goes unpaid after statements have been sent from our office - without a response from you, the account will be forwarded to a collection agency. There will be an additional 30% collection charge added to your account to recover the charges incurred by the collection agency. Late payment reporting on your credit report and collection process will begin and the account will be controlled by that agency. Please contact us with any questions of responsibility PRIOR to this action.
